I give permission to the Monta Vista Sports Camp staff to act for me according to their best judgement in any emergency requiring medical attention. I also certify that my son/daughter has medical insurance and is able to participate in the activities described. I also understand that campers can be dismissed for misconduct or unsocial behavior. If you have any special medical concerns, please send a note with the limitations to MV Sports Camp /936 White Cloud Dr. / Morgan Hill, CA 95037.
